b.) Original proof of residence indicating the name &
     address dated at least 1 month prior loan application
     (electricity, water, telephone, postpaid mobile phone, cable)
c.) Certificate of Employment (for employee/affiliate)
d.) Latest 3 months bank statements (current/savings account)
     (for private practitioner) and/or 1 month pay slip (for employee/affiliate)
